Integrating Custom Forms with Calendars

Overview

Integrating custom forms with your PXME calendars ensures you collect all the essential information before clients book. This not only streamlines the booking process but also reduces back-and-forth communication and prepares your team with the details they need in advance. Forms can capture standard contact info, location details, and custom fields such as referral source or reason for visit.


Step 1: Create a Custom Form

  1. Go to Sites > Forms in the top menu.

  2. Click Add Form and either build from scratch or use a template.

  3. Name the form clearly for easy identification.

  4. Remove any unnecessary fields (such as default consent checkboxes) if they’re not needed.

  5. Add essential fields:

    • First Name, Last Name, Email, Phone (required)

    • Address, City, State, ZIP (optional but useful for local practices)

  6. Add custom fields for your needs, such as:

    • “Reason for Visit” (dropdown/text)

    • “Referral Source” (dropdown: Google, Facebook, Friend, etc.)

    • “Child’s Age” (number/dropdown)

  7. Mark required fields where necessary and adjust layout for readability.

  8. Click Save Form when finished.

⚠️ Note: Button design comes from the Calendar Widget Customization settings, not the form itself.


Step 2: Link the Form to Your Calendar

  1. Go to Calendars > Calendar Settings.

  2. Select the calendar you want to edit.

  3. Open the Forms & Payment tab.

  4. From the dropdown, choose the custom form you created.

  5. Adjust the order:

    • Show the form before the booking widget to capture details upfront.

    • Or show the widget first if you prefer clients to select time before filling details.

  6. Choose post-booking behavior:

    • Display a thank-you message, or

    • Redirect clients to a funnel/website page.

  7. Click Save to confirm.


Step 3: Test the Integration

  1. From Calendar Settings, click the Share (arrow) icon.

  2. Open the booking link in a new tab.

  3. Confirm:

    • All form fields display correctly.

    • Required fields cannot be skipped.

  • Keep forms short — too many fields discourage completion.

  • Use custom values to pre-fill fields and create smoother booking.

  • Validate inputs like email, phone, and date to ensure accuracy.

Yes. Any fields you create in the form builder can be included when linked to a calendar.

Q2: How do I confirm my form is linked correctly?
Go to Calendar Settings > Forms tab and verify your form is selected. Test the booking link to double-check.

The form won’t submit until all required fields are completed.

Q4: Can I edit a form after it’s linked?
Yes. Edits made in Forms automatically update in the linked calendar.

All form submissions are stored under Forms Submissions in your PXME dashboard.
